Our Client, a Healthcare company, is looking for a Registered Nurse - Case Manager for their Apple Valley, CA location. Responsibilities: Participate in the development of the treatment plan. Requirements: Extra checklist required Must Have Hospital experience, Behavioral health experience does not apply Experience with Assessing patients, Care Coordination, Discharge Planning and Utilization Review Ability to organize and participate in family meetings. Experience with HMOs, IPAs, etc. Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, advance directive, POLST, protective services process, etc. Competent with arranging DME, SNF and ambulance transport.
