Job Description:
A company is looking for a Senior Care Options RN Care
Manager.
Key Responsibilities
Conduct telephonic outreach and face-to-face assessments to develop
and implement member-specific care plans
Provide health education and coordinate care transitions to ensure
safe post-hospital discharge
Collaborate with interdisciplinary teams to improve member outcomes
and maintain professional development
Qualifications
Registered Nurse with a current unrestricted license in the state
of residence
Bachelor's degree or relevant equivalent experience; preferred in
nursing
Minimum of 5 years' relevant clinical experience; home care or case
management experience preferred
National certification in Case Management is desirable
Proficiency in a second language is desirable
