Social Worker - Social Services



Location: Apple Valley, CA


Work Schedule: Full time - 80 Biweekly Hours


Shift: 8-hour, Days



Provides psychosocial assessment, crisis intervention, referrals and information to patients and families. Provides education/support services to patients, families and staff. Works collaboratively with the interdisciplinary team to effect positive patient and family outcomes and to ensure that resources are in place to facilitate the discharge plan. Serves as resource for less experienced social workers.



Minimum Requirements:

• 
  
• Master's Degree in Social Work from accredited university.
  
  
• Coursework/Training - Courses or continuing education in Evidence-Based Intervention.
  
  
• 1 year Social work experience in a health care setting (outpatient/acute care or skilled nursing facility).
  
  
• Registered with CA Board of Behavioral Sciences as an ASW due within 90 days of hire.
  
  
• LCSW Designation preferred by evidence of Primary Source Verification.
  
  
• LCSW licensure with BBS due within 4 years of hire.
  
  
• BLS Certification.
