Description RN Manager of our Care Management Team at Providence St. Mary Medical Center in Apple Valley, CA. This full-time leadership position will oversee the staff and all areas of Care Management to include a manager on-call responsibility every other week for emergent issues. Case Management is a collaborative practice including patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. The Case Management process facilitates communication and care coordination along a continuum through effective transitional care management. Recognizing the patient's right to self-determination, the significance of the social determinants of health and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.). Required Qualifications: - Bachelor's Degree Nursing - Upon hire: California Registered Nurse License - 5 years Previous experience working in a healthcare role. - 3 years Previous Case Management experience within an acute care setting. - 1 year Healthcare leadership experience. Preferred Qualifications: - Master's Degree Masters in Healthcare or related field - Upon hire: Certified Case Manager (CCM) or Accredited Case Manager (ACM) Why Join Providence?
